Surrey Heartlands survey on self care

Surrey Heartlands Health and Care Partnership are conducting a survey on self care and would like to know how you feel about looking after your health and wellbeing. 

The survey will help Surrey Heartlands Health and Care Partnership to make improvements to health and social care services across Surrey and it can be completed even if you do not have a medical diagnosis because self-care is:
– how we live our lives day to day
– how we look after ourselves or our family when we get a minor illness like a cold
– how we manage a long term condition
– how we recover after being in hospital

The survey should take 10-15 minutes and responses will be kept anonymous.
