Smokefree policy questionnaire

Surrey & Borders Partnership (SaBP) would like people who use their services to complete a short questionnaire on their Smokefree policy.
The Trust is in the process of reviewing the current Smokefree policy and recognises the importance of gaining feedback from those most impacted. As such, the questionnaire is intended for people who have had a hospital inpatient stay since October 2017. The responses to this questionnaire are anonymised and closes on Friday 12 July.
