Have you recently been discharged from Epsom & St Helier hospital? – ends 30th October
The ARC KSS are undertaking a research project about the Discharge to Assess pathway currently being run in some areas. This is when someone is discharged from an acute hospital back into the community (their own home, a care home or a rehabilitation centre).
If you have recently been discharged from hospital, or supported someone who has been discharged from particular hospitals (Surrey – The Epsom & St Helier Hospital, Sussex – Any hospital in the East Sussex Healthcare Trust, Kent – Darent Valley Hospital), they are interested to hear your experiences of what this was like. This will take place in a 45 minute interview over the phone or online.
